Below you can download and print copies of the release/waiver form, medical form, and photo release forms. Every participant will need to have the release/waiver form and medical form filled out prior to participation on a Northwest College challenge course program. We would also appreciate all participants having the photo release form signed, for use of photos on the website and printed material.
